Before Your First Appointment
Preparing for your first visit:
Helping your child understand what will occur at my office:
Tell your Child that Dr. DeFrancisci Lis or “Dr. D” would like to find out how she/he learns, thinks, and feels so she can work together with the family to find solutions to problems that are getting in the way to being more successful at school, home, or with friends.
Please arrive 15 minutes early for your appt time
Please fill out the Life History Questionnaire/ and have teacher fill out the SNAP FORM available on my website prior to the first appointment and bring these forms with you.
For younger children it is very important you bring someone who can stay with him/her while you are with Dr. DeFrancisci Lis or notify her ahead of time so a separate appt time can be made with the parent.
Please read print out and sign the Guide to Clinical Services.

For children please print out the Teacher Feedback Form and the SNAP Form and give the forms to the teacher prior to be filled out prior to the first appointment.
